YOUR FIRST VISIT & WHAT TO BRING

We want to welcome you to our practice and take this opportunity to give you some information to help you on your first visit.

Our physicians see children from infancy through adolescence.  We work hard to provide same day appointments for your sick or injured child.    Appointments for less urgent issues or chronic conditions are typically scheduled in advance.  While we schedule Well care visits in advance, they can often be accommodated sooner if you are willing to be flexible about the doctor that you see.

We offer separate well and sick waiting rooms.  Messages left are returned that day and after office hours the on call doctor will return your call.

At your first visit, we recommend that you print and complete the following forms at home.  These forms can be faxed to our office at 661-294-8399 prior to your child’s scheduled appointment.  Please be sure to complete all fields on each form.

Also, in order to save time at your visit we strongly urge you to have your child’s medical records and immunization records sent to us prior to your child’s scheduled appointment.  To request these records from a previous physician, please print and complete the following medical record release form and send it to your previous pediatrician. Please be sure to complete all fields.

Patient Registration  (for ages newborn - 17 years) Please send all completed forms to info@valpeds.com prior to your scheduled appointment.

Optional – If you want to give permission for someone other than you (the legal guardian) to bring in your child for care:

Please arrive 15 minutes earlier than your appointment time.  This allows you to find parking, check in, and have our staff review your registration forms.

Please bring your insurance card, and any other medical and immunization records you have.  Insurance Information can be found here.

If you would like to transfer your child’s records over to us ahead of time, print and fill out our medical record release form and mail it to your previous pediatrician.
